From July 1, 2021, a new report has been introduced in Moscow and the Moscow Region: on the number of vaccinated employees.
Companies and individual entrepreneurs in Moscow and the Moscow Region must confirm that they comply with the mandatory vaccination requirements for employees. To do this, it is necessary to submit in electronic form the relevant information in the form approved by the regional authorities.
Who submits the report
The vaccination report is submitted by companies and individual entrepreneurs, whose employees must undergo compulsory vaccination against coronavirus infection. There is no exact list with the name of the companies and individual entrepreneurs. It is necessary to be guided by the .
It is necessary to submit a report both for employees with whom they have entered into an employment contract (including part-time employees), and for employees who perform work or provide services under civil law contracts. Information form
Employers of Moscow have to send information in the form specified in Appendix 2 of the decree of the Mayor of Moscow dated June 16, 2021 No. 32-UM. An example of filled report by companies and entrepreneurs in Moscow (see template).

Deadline
For employers in Moscow, the deadline for submitting information is from July 1 to 15, 2021. Information must be submitted in electronic form through the on the official website of the Mayor of Moscow. The service for submitting information will be available from July 1, 2021 (clause 14 of the decree of the Mayor of Moscow dated June 16, 2021 No. 32-UM).
In the Moscow region, the deadline for submitting information is also from July 1 to 15, 2021. Information must be submitted in electronic form through the personal account of a legal entity or individual entrepreneur on the . The service for submitting information will be available from July 1, 2021 (clause 2.2 of the Decree of the Governor of the Moscow Region dated June 16, 2021 No. 184-PG).
Vaccination data
For each employee who has been vaccinated against coronavirus, the information indicates SNILS number, CMI policy number, if any, the series and number of the identity document or patent for foreigners, the mobile phone number. It is not necessary to indicate full name.
An employee can confirm vaccination by the certificate of vaccination against coronavirus. The paper certificate contains the dates of receiving the vaccine, its name and series. It also contains the signature of the doctor and the seal of the medical organization.
To get the information you need from employees, make a request to each employee asking for this information. Make a request in any form. If the employee agrees to provide information, then in the request he/she immediately indicates his/her personal data and puts the signature. If the employee is absent from work or works remotely, then send the request by e-mail. The employee can also send you information in response by e-mail.
When you send information to the Moscow City Government or the regional portal of the Moscow Region, you do not need to take a separate consent to the transfer of personal data from each employee. The processing of personal data of employees does not require consent when the obligation to transfer information is established by law (Article 6 of the Federal Law of July 27, 2006 No. 152-FZ). The decree of the Mayor of Moscow and the decree of the Governor of the Moscow Region were issued within the framework of federal legislation, which makes it possible not to take the consent of employees to transfer their personal data.
